The mission of Community Health Center of Cape Cod is to improve the health status of the patients and community we serve through the provision of comprehensive, high-quality, compassionate health care to all, regardless of their ability to pay.
Community Health Center of Cape Cod (CHC) is a patient-centered medical home providing integrated primary care, dental and behavioral health services to the most vulnerable children, adults and families on Cape Cod.
CHC is unique in the fact that we provide care to more than 16,000 individuals within a "complete medical home" model, where multidisciplinary, integrated health services are provided under one roof to assist patients overcome transportation, affordability, financial, linguistic, cultural and other barriers to care. Patients who visit can receive primary care, behavioral health care, optometry, dental services and women’s health care, as well as specialized services and a full-service pharmacy. In October 2013, CHC was awarded Level 3 (the highest level) Recognition as a patient-centered medical home (PCMH) by the National Committee for Quality Assurance.
Many patients the Health Center cares for are the working poor: 80% of the population served is at 200% of poverty level or below. Health Center patients come from every town on Cape, along with Martha’s Vineyard and other towns in Southeastern Massachusetts. CHC cares for all those in need of services, especially the uninsured, underinsured and those covered by public insurance. 52% of Health Center patients have MassHealth or another state insurance plan, 22% have Medicare coverage and 5% have no insurance.
